DIGESTIVE HEALTH TEST

This Digestive Health Test lists symptoms and other factors most commonly found in people suffering from some form of allergy. Read each question carefully and check the boxes that apply to you. When you finish, submit the questionnaire and the form will calculate your score.

 

Put a check in the box for any symptoms that you have

1. Take pain killers and/or anti-inflammatory medication (aspirin, cortisone, ibuprofen, Aleve) more than once per week?

2. Have you taken more than two round of antibiotics in your life?

3. Have you experienced gastrointestinal problems, i.e., ulcers, diverticulitis, irritable bowel, lactose intolerance, diabetes, colitis?

4. Have you ever had a problem with yeast overgrowth such as athlete’s foot, jock itch, or for women, vaginal yeast infections?

5. Do you have (or have had) Heartburn greater than once per month?

6. Do you eat small, frequent meals to avoid indigestion and bloating?

7. Do you avoid eating fats?

8. Do you experience abdominal discomfort and/or distention?

9. Do you experience belching, burping, or bloating?

10. Do you experience flatulence?

11. Do you experience indigestion due to strong feelings, stress at home or at work?

12. Do you experience abdominal pain and/or cramping?

13. Do you experience relief from abdominal pain after a bowel movement?

14. Do you have persistent diarrhea or constipation or both?

15. Do you pass undigested food?

16. Do you experience less than one bowel movement daily?

17. Do you have hard, dry stool?

18. Do you depend on laxatives (stool softeners, mineral oil, herbs)?

19. Do you have bad breath?

20. Do you have diminished interest in food and general lack of appetite?
